The Division of Occupational Safety and Health (DOSH) is investigating the death of a Safeway Stores employee at the company's milk processing plant in San Leandro.The worker, 42-year-old Anthony Fraga of Fremont, was crushed when a hydraulic milk crate pallet loader on which he was performing maintenance activated unexpectedly in the early morning hours of Jan. 21.The DOSH investigation will focus on possible lockout/tagout violations, as well as the company's training program, acc
